29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: a.) On or about April 19, 2020 at 102-01 66th Road, Forest Hills, NY 11375 An employee working as a Patient Care Associate died on April 19, 2020. On or about April 19, 2020, the employer was notified that the employee had died after exposure to SARS-CoV-2, the virus that causes COVID-19. The employer failed to report the death to OSHA within eight (8) hours of learning of the fatality. ABATEMENT CERTIFICATION IS NOT REQUIRED
