29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident (a) Site: The employer failed to report the death of an employee who was mortally injured while cleaning a weapon at the site location within the allotted 8 hr. time frame. The incident which occurred on May 01, 2020, approximate time 2:30pm was not reported to the Atlanta West Area Office until May 06, 2020 approximate time 10:00 am, 5 days 19 1/2 hours later.
