29 CFR 1926.502(i)(4): All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: (a) Brox Construction, LLC at 16191 E 40th Ave. Denver, CO 80239: On or before 4/30/2020 the exposing employer did not ensure that any and all hole covers were marked in accordance with this standard. An employee fell through a 4 foot by 8 foot hole which had been covered with a sheet of plywood. This condition exposed employees to a fall hazard of 24 feet.
