29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: On or about April 15, 2020, inside the bowl of the 250,000-gallon multi-column elevated water storage tank, the employer did not ensure employees working within the tank bowl were protected from falling more than 6-feet through the riser pipe hole to the lower surface below.
