29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about March 27, 2020 and at times prior thereto on the roof of Mill L, employees unclog filters with a metal rod through R-Valve #17 clean-out opening which has one of the metal bar safety guards cut off. This condition exposed employees to amputation hazards from rotating parts in the R-Valve.
