29 CFR 1904.39(a)(1): Within eight (8) hours after the death of any employee as a result of a work-related incident, the employer did not report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor: a) 1733 Spruce St, Philadelphia, PA - On or about March 27, 2020. The employer did not report the fatality of an employee in the workplace within 8-hours. The incident occurred on February 11, 2020, the employee died on February 13, 2020 the employer did not contact OSHA of the fatality until February 20, 2020.
