29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: a- On 3/3/2020 an Optimize Manpower Solutions employee performing NJ State-contracted housing inspection work sustained injuries within an apartment at 144 Kaine Avenue in South Plainfield, NJ 07080 and died the following day. Notification of the work related death was not made to OSHA within 8 hours of the occurrence. Condition noted on or about 3/16/2020.
