Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.22(a)(1): All walking-working surfaces were not kept in a clean, orderly, and sanitary condition: On or about 3/17/2020 at the mezzanine area, workers were exposed to slip and fall hazards of up to seven feet to the lower surface where the walking/working surfaces were not maintained in a clean condition where mixing materials such as but not limited to calcium were observed filling in the non-skid treads and surfaces of equipment and areas including but not limited to: a. Werner eight foot tall step ladder; b. Werner six foot tall step ladder; c. Premier platform (Scissors) lift; d. stairs accessing and egressing from the mezzanine; and e. Mezzanine flooring separating creating a half inch tripping hazard.
