29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs to provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer. On or about March 12, 2020 and prior to that time located on OH-28 between 15751 OH-28 and US-50 Chillicothe OH 45601 for a short duration for a mobile utility construction work crew in which the employer opted to utilize temporary traffic control work zone flagger stations and flaggers on a two lane rural 45 mph roadway there was no frequent and regular inspection program being maintained in use by a competent person in that the companys established onsite inspection safety program of temporary traffic control zone(s) job site, materials and equipment were not being planned, designed and monitored in detail necessary to conform with the standards, options and guidance of MUTCD 2009 edition Chapter 6E and Temporary Traffic Control Plans.
