Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were using a hook that lacked a functioning safety latch, thereby, exposing employees to a struck by hazard. a) Production Area: Employees were using a CM Hurricane hand operated chain hoist, with attached hook, rated for 1 ton, without a functioning safety latch installed on the hook, to lift paper rolls, weighing in excess of 1000 pounds. The employer did not ensure that the safety latch on the hook, used to lift paper rolls, was functional, per ANSI B30.16-2012, Section 16-1.2.9 (hooks), thereby exposing employees to a struck by hazard, on or about 3/11/20 & 3/17/20.
