29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: At 14530 NW 112 Ave, Hialeah Gardens, Florida, 33018: On or about March 13, 2020 and at times prior, an employee was exposed to a fall hazard of 13 feet to ground level, while walking and working on top of a conveyor belt and not using any means of fall protection.
