29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: At the worksite located at 105 South Highway 5, Browning, Missouri 64630, employees were exposed to serious amputation and caught-in hazards caused by rotating parts from the Kelly Duplex Corn Crimper, serial number 55361. The employer did not ensure that employees who were conducting clean out operations of the Kelly Duplex Corn Crimper, serial number 55361, first utilized the equipment specific procedures to lock out and tag out the machine from its source of energy.
