Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation were performed by the customer or user without manufacturer's prior written approval: Plant - On or about 03/04/20 and at times prior thereto, powered industrial trucks including (but not limited to) the Doosan 25 LPG-powered forklift were operated with the front racks removed. Or in the alternative 29 CFR 1910.178(q)(6): Industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, or were altered either by the addition of extra parts not provided by the manufacturer or by the elimination of parts, except as provided in paragraph (q)(12) of this section. Plant - On or about 03/04/20 and at times prior thereto, powered industrial trucks including (but not limited to) the Doosan 25 LPG-powered forklift were operated with the front racks removed. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
