29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section: (a) Protecto Wrap at 2255 S. Delaware St., Denver, CO 80223: On and preceding February 28, 2020, the employer did not establish clear and specific procedures to outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy in that the instructions concerning thermal energy control were not documented or discussed prior to conducting the task. Employees from maintenance were exposed to the hazards of inadequate lockout procedures during repair of a thermocouple housing on the side wall of the Smart tank. This condition exposed employees to hazards associated with equipment or machines that contain multiple sources of energy capable of injuring employees while performing servicing, cleaning, and maintenance activities resulting an employee receiving thermal burns to the head and body when hot asphalt unexpectedly released from this equipment.
