29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): On or about March 5, 2020, during the installation of roofing material on a residential single family home located at 7947 Chatham Ave NW, N. Canton, Ohio 44720, the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards (no fall protection while working on the roof) (portable ladders did not extend 3 feet above the upper landing surface) and struck by hazards (employees working installing material with air driven nail guns) from working surfaces of heights approximately 10.1 feet.
