29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation: (a) On or about Feb 21, 2020, and at times prior, for the employees exposed to cave-in and struck-by hazards while repairing a water supply pipeline in a trench greater than nine feet in depth where water flowed from the pipeline completely covering the bottom of the trench, in the vicinity of 350-420 66th Street SE, Bismarck, North Dakota.
