Operator(s) were not trained and instructed in the safe method of work before starting work on mechanical power press(es). The employer did not insure by adequate supervision that correct operating procedures were being followed. On or about February 19, 2020 an employee was assigned to work on a punch press and was not adequately trained and supervised on how to run the machine safely. The employee was exposed to amputation hazards while operating the punch press. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
