29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Dock- On February 26, 2020, the employer did not obtain the manufacturers written approval prior to modifying the Crown Electrical Mule, Model #TR 4500-250, and the Toyota Electrical Tow Truck, Model #8TB50-41984 by welding a rest bar attachment behind the backrest.
