29 CFR 1910.147(e)(3): Each lockout or tagout device was not removed from each energy isolating device by the employee who applied the device: On or about February 18, 2020 and at times prior thereto, MJ Saw Helpers that affixed lockout devices on the MJ Saw did not maintain security of lockout device keys and allowed other helpers to remove their affixed lock, exposing employee(s) to hazards of unexpected start-up of energy sources.
