29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a. By shipping docks 1-4 in the southeast corner of Sunbelt Chemical Corp.'s facility: On or about February 21, 2020, and at times prior, the employer exposed employees to struck by hazards by failing to ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, in that an employee was struck by a forklift that resulted in the employee's hospitalization and right leg amputation.
