OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee was exposed to potential crushed by/struck by hazards in the event of the tip over of an aerial lift. 171 Main St., Westfield, MA: on or about February 25, 2020, an employee was observed using a Genie S-65 aerial lift contrary to the lift manufacturers recommendations to transport wooden railings to the roof of the residence being remodeled at this address. The Wooden rails were laid across and overhanging the rails of the basket of the lift which exposed the employee to serious hazards resulting from unexpected tip over of the lift and/or unexpected load movement that could strike or entrap the lift operator. Among other methods, feasible and acceptable means to correct this hazard is: 1. Ensure that employees follow the manufacturers recommendation to not place or attach overhanging loads to any part of this machine 2. Ensure that employees are trained and follow the instructions posted on the body of this machine which under a Danger warning, which states, "Do not place or attach overhanging loads to any part of this machine". 2.
