At the workplace located at 127 Claxton Avenue, Carthage, Missouri 64836, employees were exposed to struck-by, caught-in and amputation hazards while operating the Hayssen 1 Shred Johnson Cutter due to unexpected machine operation while body parts were inside of the point of operation and/or under the pneumatic guard. Among other methods, one feasible and acceptable abatement method is to: a) Guard the foot pedal to prevent unintended operation in accordance with American National Standard ANSI B11.3-2012. b) Provide a two-hand method of actuation available for operator use.
