OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish to each employee employment and a place of employment which was free from recognized hazards, which were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by hazards while operating a crane: a. On or about February 21, 2020 in the architectural shop, employees were exposed to struck-by hazards when the hoist control station operating the Gaffey 10-ton crane, did not have function labels. b. On or about February 21, 2020 in the architectural shop, employees were exposed to struck-by hazards when hoisting material using the Gaffey 10-ton crane without a functioning safety latch on the hook of the southwest hoist. c. On or about February 5, 2020 in the machine shop, employees were exposed to struck-by hazards when hoisting material using the western ProservAnchor 10-ton crane without a functioning safety latch on the hook of the southeast hoist.
