29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) M.A. Mortenson Company at 1600 12th Ave. Denver, CO 80204: On or before 2/19/2020 the controlling employer did not ensure that all areas of the jobsite were inspected regularly to ensure employees and subcontractor employees were not exposed to serious hazards. Superintendents and safety personnel did not ensure that the area where the cisterns were located was free from fall and drowning hazards. Nine manholes were not guarded during and after concrete was poured around the manholes. A subcontractor employee fell through an unguarded manhole and into a cistern filled with water. The subcontractor died as a result. These conditions exposed employees and subcontractor employees to fall and drowning hazards.
