29 CFR 1910.119(e)(6): 1910.119(e)(6): The employer did not update and revalidate the requirements in paragraph (e)(4) of this section at least every five years, to assure that the process hazard analysis is consistent with the current process. a.) Combined Systems, Inc. - On or about February 14, 2020, and at times prior thereto: employees were potentially exposed to fire and/or explosion hazards in that, the employer had not updated and revalidated the process hazard analysis (hazard evaluation), at least every 5 years, pertaining to the process of manufacturing delay tubes for flash bang grenade devices consisting of Hazard Class/Division 1.1, 1.3, and 1.4 explosive material. The process hazard analysis for manufacturing delay tubes, at the Jamestown, PA facility, was last updated and revalidated on or about August 18, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
