29 CFR 1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On 2/7/2020, employees of Aarrow Boring, Inc. operating as a subcontractor for Unnerstall Contracting Company LLC were performing boring operations at 11660 Administration Drive, Saint Louis, MO. The employees were exposed to struck-by and engulfment hazards inside a trench which was dug by Unnerstall Contracting Company LLC. Frequent and regular inspections of the job site, materials, and equipment were not performed.
