29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)[A], (c)(4)(ii)[B], (c)(4)(ii)[C], and (c)(4)(ii)[D]: On February 12, 2020, the employer failed to clearly identify all of the specific steps to be followed by employees to isolate and disable the Titan Line Shaft conveyor along the "Big Line" in order to safely de-energize the equipment and control the energy hazards and to prevent unintentional exposure to hazardous energy. The current procedures lacked the following: 1. The purpose, scope and authorization of the energy control procedure; 2. Identify and locate energy disconnect switches and/or valves to control isolation; 3. Sequential steps to be followed to protect employees from hazards; 4. Identify the steps to take in order to ensure residual and/or stored energy has been removed; 5. Specific steps to verify and test that de-energization and isolation has been accomplished; 6. Provide steps for restart.
