29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) Travel lane area of work yard: On or about 02/7/2020, and at times prior, the employer exposed employees to struck-by hazards, in that they were using a clamp attachment on a Yale propane forklift without the prior approval of the manufacturer. The forklift and attachment were used to lift and carry bags of recycled tire materials to be loaded on trucks.
