Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer does not furnish to each of his employees employment and a place of employment which are free from recognized struck-by and caught-in hazards that are causing or likely to cause death or serious physical harm to his employees: a) This violation occurred on or about January 29, 2020, at a workplace located at 5033 Lemon Ave., Dallas, TX; where an employee, who was in a scissor lift installing a light fixture on an overhead roll-up door, was not protected from the unexpected startup of the automatic roll-up door. The employer by not ensuring that the automatic roll-up door was isolated from its energy source and rendered inoperative exposed the employee to struck-by and caught-in hazards.
