29 CFR 1904.29(a): The employer must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. (a) Scott Waneka Inc. dba S.W.I. Excavating at 2278 S. Delaware St, Denver, CO: On or before February 6, 2020, the employer did not log an employee recordable injury that occurred on 10/3/2019 on the 2019 Log of all Work-Related Injuries and Illnesses (OSHA Form 300), and the 2019 Injury and Illness Incident Report (OSHA Form 301).
