29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about 2/6/2020, at Blake & Pendleton workplace located 269 North Street, Macon, GA: Employees were exposed to struck-by and caught-between hazards, in that the employer failed to have machine specific procedures to lockout the powered industrial truck(s) gravitational (Kinetic) and electrical energy sources were not utilized. IN THE ALTERATIVE 29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about 2/6/2020, at Blake & Pendleton workplace located 269 North Street, Macon, GA and various field site location in GA: Employees were exposed to struck-by and caught-between hazards from energy sources such as but not limited to electrical, gravitational (Kinetic). The employers alleged procedure for various manufactured powered industrial trucks not complying with all lockout tagout procedure requirements. Procedure were not written or available in sufficient detail and provide enough direction so that each authorized employee can follow the procedure and determine how to safely perform servicing or maintenance work.
