29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a. On or about 11:30 AM on November 28, 2019, at 400 Corbett Street, Belleaire, FL, an employee suffered a work-related injury and was hospitalized. The employer was aware of the in-patient hospitalization on November 29, 2019 at around 8:30 A.M. The employer notified OSHA on December 2, 2019 at 2:45 P.M. of the in-patient hospitalization. Baycare Health System, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1904.39(a)(2) which was contained in OSHA inspection number 1105357, citation number 1, item number 1 and was affirmed as a final order on 12/29/2015, with respect to a workplace located at Morton Plant Hospital, N Bay Recovery Center - 21808 SR 54 Lutz, FL 33549.
