29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: a) On January 29, 2020, the employer failed to fully document its training of employees operating the mill elevator with regard to responses to alarms in the elevator control room. b) On January 29, 2020, the employer failed to include a permit required confined space sign outside an elevator leg bonnet where the leg and bonnet were bolted shut.
