29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: At a worksite located at 17652 Gum Rd, Carthage, MO: Employees were exposed to fall hazards. This was demonstrated on January 29, 2020 and at times prior when an unprotected side or edge was located in the third floor mezzanine. The employer failed to ensure that employees were not exposed to fall hazards greater than 4 feet by installing protective systems or use of personal fall protection.
