29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about January 20, 2020, the employer failed to ensure employees unjamming the former infeed system, transfer arms, were protected against crushing injuries. The employer did not utilize their LOTO procedures for the former.
