29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about January 22, 2020, at the 39 Line, employees were exposed to struck by and caught-in injuries when the employer did not ensure that the Let-off machines including, but not limited to 11 and 34 were guarded to protect the operator from rotating pulleys, moving wires, and in-going nip points from rotating reels. b. On or about January 22, 2020, at the 39 Line, employees were exposed to struck-by and caught in injuries when the employer did not ensure that the Take-up machines including, but not limited to 33, 32, and 31 were guarded to protect the operator from rotating pulleys, moving wires, and in-going nip points from rotating reels. c. On or about February 20, 2020, at the 39 Line, employees were exposed to caught-in injuries when the employer did not ensure that the Take-up machines including, but not limited to 31 and 32 were guarded to protect the operator from in-going nip points at belts and pulleys of the level winder arms.
