29 CFR 1910.178(a)(4) [as referenced in 29 CFR Part 1915.120] : Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. (A) 7 Mosquito Lane, Tuckahoe, NJ-Main yard: Modifications and additions were made to the Doosan forklift without manufacturers prior written approval as required. Condition observed on or about January 23, 2020. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
