29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about January 15, 2020 the employer failed to ensure that employees performing service or maintenance on the R560 vacuum packaging line operated the controls in a manner which isolated the unit from hazardous energies. Hazardous energies present on the equipment include, mechanical, electrical, pneumatic, thermal, and vacuum.
