29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part: (a) At the demolition site, on or about January 6, 2020, and at times prior to, employees were exposed to a crush-by hazards when the employer did not initiate and maintain an accident prevention program and/or safety and health program to comply with the regulations of this part which included, but were not limited to, employee protection when exposed to hazards associated with free falling equipment, as well as bracing and/or supporting mechanical equipment, during demolition / de-construction activities.
