29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) 2618 Route 302 Middletown, NY- On or about 12/19/2019, employees were exposed to a fall hazard of thirty feet through unprotected skylight openings without covering them or by using fall protection while working on the roof.
