29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Grinder Machines: On or about 10/4/2019, and at times prior, the employer exposed employees to amputation hazards in that the employer did not develop lockout tagout procedures to control hazardous hydraulic energy while performing tasks such as breaking down the grinder equipment on line 77 or 76.
