29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) Ruscilli Construction Company Inc. located at 230 E. Long Street, Columbus, OH 43215: On and preceding December 10, 2019, the controlling employer did not ensure employees were protected from tripping in or stepping into or through holes by covers. Floor holes ranging from 2 (two) to 11 (eleven) inches at the least dimension were in the path of workers travel throughout the 3rd floor, 4th floor and courtyard area. The following condition exposed employees to tripping and fall hazards.
