29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about 12/09/2019, an employee with John Volpi & Company, located at 5263 Northrup Ave., St. Louis, MO, was exposed to point of operation hazards which caused an amputation of the middle, ring and little fingers of the right hand. The following machine did not have an adequate guard: The Reiser Line #2 Repack crosscut machine (Model: RE20/6, S/n: 2060060, 220 V) did not have an adequate guard to protect at the point of operation.
