29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Conveyor between debarker and chipper machines: On or about November 25, 2019 and times prior thereto, the employer did not ensure lockout tagout was used while employees were walking and working on the conveyor's chain belt while conducting activities related to unjamming logs which exposed employees to injuries associated with the inadvertent startup and movement of the conveyor. b) Chipper machine: On or about November 25, 2019 and times prior thereto, the employer did not ensure lockout tagout was used while employees were walking and working on the conveyor's chain belt while conducting activities related to unjamming logs which exposed employees to injuries associated making contact with the chipper due to the inadvertent startup and movement of the conveyor.
