29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident. An employee was hospitalized on October 28, 2019 with a work-related injury and died November 25, 2019. The employer was notified of the death on November 29, 2019 and did not report the fatality until November 30, 2019 after 8-hours had passed.
