29 CFR 1926.20(b)(2): "Accident prevention responsibilities." Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. (a) On or about Nov. 22, 2019, located at the 6th floor of a parking garage structure, there were no frequent and regular inspections being performed by a competent person in that employees erected formwork, that later collapsed during a concrete pour, while using the same shoring layout in the area of a 24 inch slab thickness as that used in areas of 8 inch slab thickness. (b) On or about Nov. 22, 2019, located at the 6th floor of a parking garage structure, there were no frequent and regular inspections being performed by a competent person in that employees adjusted the height of the formwork, that later collapsed during a concrete pour, in order to accommodate the 24 inch concrete thickness of the 7th floor slab fold area without determining the correct form spacing. (c) On or about Nov. 25, 2019, located at the 6th floor of a parking garage structure, there were no frequent and regular inspections being performed by a competent person in that employees erected formwork, that later collapsed during a concrete pour, that was not inspected by a competent person immediately before and during placement of wet concrete.
