29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) 140 Evergreen Drive, Bradford MA 01835 On or about 9/30/19, an employee fell from height, and was hospitalized as a result. The employer was notified of the hospitalization on 10/1/19, however, the in-patient hospitalization was not reported to OSHA until 10/9/19.
