29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a. On or about November 6, 2019, in front of the residential building, an employee operating a mortar mixer was exposed to amputation hazards and was injured. The employee has not been trained in the proper use of the equipment and the hazards associated with the operation of the equipment.
