29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about October 29, 2019 at 12:15 p.m., an employee suffered a work-related injury, and was admitted at the hospital for surgery. The employer was aware of the injury on October 29, 2019. The employer notified OSHA on November 6, 2019 at 3:21 p.m.
