29 CFR 1910.28(b)(15): The employer did not ensure each employee on a walking-working surface not otherwise addressed by other subparts of this part which are 4 feet (1.2m) or more above a lower level were protected from falling by guardrail systems; safety net systems; or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems. (a) Plastics Department Injection Molder - On or about 10/25/19, the employer did not ensure that each employee standing on top of a Uniloy structural foam machine was protected from falling, exposing employees to fall hazards.
